How to register as a new patient online
We are using a new online registration service for new patients that makes it easy to register with at Frome Valley Medical Centre.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.
Registering as a new patient with a PRF1 paper form
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. If you are already registered at the Surgery and you move house but still to a location within our Outer Boundary, you can stay registered with us. However, as a new patient, we will not be able to register you if you live within the Outer but not Contractual Boundary.
If you do not have access to the internet, you will need to complete a registration form (PRF1) which needs to be signed and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
We also ask for evidence of ID to be supplied when possible as there are practical reasons why we might need to be assured that people who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us.
Forms of ID that are helpful are :
1) Photographic ID e.g. Passport/Driving licence.
2) Proof of address e.g. utility bill.
Medical treatment is available once you have provided both forms of ID and the registration has been processed. Please contact reception for further information.
Registering as a temporary patient
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Registering online as a non-English speaker

Registering children
Children under 16 need to be registered by their parent or guardian. This usually means filling in a separate form. You may also be asked to provide a form of ID to prove that you're their parent or guardian.
If you have a personal child health record (red book) from the birth of your child, it will help to provide this.
What to do if you’re registering on behalf of someone else
If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.
Change of address
If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
